Leg swelling that doesn't improve with elevation, doesn't respond to diuretics, and progressively worsens over time is not the typical venous insufficiency. It may be lymphedema — chronic swelling caused by failure of the lymphatic system, requiring a completely different treatment strategy.

What Is the Lymphatic System and Why Does It Cause Swelling?

The lymphatic system is a parallel network of fine vessels and lymph nodes that removes excess protein-rich fluid from tissues. When this system fails, fluid accumulates progressively, creating firm swelling that worsens over time if untreated.

Types of Lymphedema

  • Primary Lymphedema (Congenital): Developmental defect in the lymphatic system; often appears in adolescence or early adulthood
  • Secondary Lymphedema (Acquired): More common; results from:
    • Lymph node removal or irradiation after cancer treatment
    • Recurrent skin infections (erysipelas/cellulitis)
    • Filariasis (parasitic infection) in tropical regions
    • Trauma or surgical injury to lymph nodes

How to Distinguish Lymphedema from Venous Swelling

  • Positive Stemmer Sign: Inability to pinch the skin at the dorsum of the second toe — a hallmark of lymphedema
  • Lymphedema: Firm, non-pitting in advanced stages; begins at dorsum of foot and toes; does not significantly improve with elevation
  • Venous Swelling: Soft, pitting; begins at the ankle; improves with leg elevation

⚠️ Neglecting Lymphedema Leads to Serious Complications

Protein-rich fluid accumulation creates an ideal environment for bacteria. Recurrent cellulitis accelerates tissue fibrosis and worsens swelling. Early treatment breaks this vicious cycle.

Diagnosis

  1. Clinical Examination: Stemmer sign, swelling distribution and firmness
  2. Venous Duplex Ultrasound: To exclude venous causes
  3. Lymphoscintigraphy: The reference test for lymphatic function
  4. MRI or CT: For lymph node assessment and differential diagnosis

Treatment: Complete Decongestive Therapy (CDT)

  • Manual Lymphatic Drainage (MLD): Specialised massage that reroutes lymphatic flow
  • Compression Bandaging: High-grade compression stockings or multilayer bandaging
  • Therapeutic Exercises: Muscle contractions stimulate lymphatic pumping
  • Skin Care: Continuous moisturisation and protection from trauma
  • Surgery: In selected cases — lymphovenous anastomosis or lymph node transfer

✅ Early Treatment Makes the Difference

In early stages before tissue fibrosis sets in, significant reduction in swelling volume and quality of life improvement are achievable. The key is accurate diagnosis and consistent adherence to the treatment programme.
