For patients with end-stage kidney disease requiring hemodialysis, the arteriovenous (AV) fistula is considered the gold standard vascular access. Created surgically by connecting an artery directly to a vein — usually in the forearm or upper arm — the fistula causes the vein to enlarge and strengthen over time, making it capable of withstanding the repeated needle punctures required for dialysis three times per week.

The Maturation Period

After creation, the fistula requires 6–12 weeks to mature before it can be used for dialysis. During this period:

  • Exercise the fistula arm daily using a soft stress ball
  • Never take blood pressure measurements on the fistula arm
  • Avoid sleeping on the fistula arm or wearing tight clothing over it
  • Keep the skin over the fistula clean and moisturized

Daily Fistula Assessment

Every dialysis patient must check their fistula daily by:

  • Feeling the thrill: A continuous buzzing vibration should be palpable — its absence requires immediate medical attention
  • Listening with a stethoscope: A continuous "swooshing" bruit confirms good blood flow

⚠️ Call Your Doctor Immediately If

The thrill is absent, you notice redness, swelling, warmth or pus at the fistula site, bleeding that won't stop after dialysis, or you develop a fever with arm pain — these are emergencies requiring urgent evaluation.

Managing Fistula Complications

  • Thrombosis (clot): Treated with surgical or catheter-based thrombectomy within hours of onset for best results
  • Stenosis (narrowing): Detected by Duplex ultrasound, treated with balloon angioplasty via catheter
  • Aneurysm: Progressive enlargement requiring surgical evaluation and repair
  • Steal syndrome: Hand pain and coldness due to blood being diverted — treated surgically
  • Infection: Requires antibiotic treatment and sometimes surgical drainage

Six-monthly Duplex ultrasound surveillance of the fistula detects stenoses before they cause occlusion, allowing timely balloon treatment and preventing unnecessary access loss.
