Sclerotherapy — vein injection treatment — is one of the most effective ways to eliminate cosmetic thread veins (telangiectasias), reticular veins, and smaller varicose tributaries. Whether you have had liquid or foam sclerotherapy, the actions you take in the hours, days, and weeks following your session have a direct impact on the quality and longevity of your results. Dr. Mohamed Haggag, Consultant Vascular Surgeon in Heliopolis, Cairo, has treated thousands of patients with sclerotherapy and provides the following evidence-based aftercare guidance.

The First 24 Hours: Critical Period

Immediately after sclerotherapy, the sclerosant solution is working to irritate and seal the vein walls. Your actions during this window significantly affect the outcome:

  • Walk immediately and continue: A 15–20 minute brisk walk right after the procedure is mandatory. Walking activates the calf muscle pump, propels any residual sclerosant out of the superficial veins into the deep system, and reduces the risk of thrombosis.
  • Keep compression on: Wear your medical compression stocking or bandage without removing it for the first 24–48 hours (your doctor will specify based on what was treated). Compression keeps the vein walls in contact and improves the fibrotic reaction.
  • Do not sit or stand still for prolonged periods: Move every 30–45 minutes if you are at a desk or travelling.
  • Avoid hot baths, saunas, and Jacuzzis: Heat causes vasodilation, which can reopen treated veins and reduce the sclerosant's effectiveness.

The First 2 Weeks: Protecting Your Results

  • Continue wearing compression stockings during the day for 2–4 weeks — the duration recommended by Dr. Haggag based on the extent of treatment
  • Avoid direct sun exposure on treated areas. The skin overlying treated veins is photosensitive — sun exposure during healing causes hyperpigmentation (brown staining) that can persist for months
  • No vigorous exercise (running, gym, high-impact aerobics) for 7–10 days. Walking and gentle cycling are fine.
  • Avoid long-haul air travel for 2 weeks after foam sclerotherapy of larger veins
  • Do not scratch or massage treated areas — the inflammatory response is intentional and part of healing

⚠️ Signs That Require Medical Review

Contact Dr. Haggag if you notice: a tender, hard lump that is enlarging (trapped blood — this is common and can be aspirated to prevent staining); spreading redness or warmth suggesting infection; visual disturbance, headache, or chest symptoms after foam sclerotherapy (rare but requires urgent assessment); significant swelling of the whole leg.

What to Expect During Healing

Understanding the normal healing process reduces anxiety and prevents patients from mistakenly believing the treatment has failed:

  • Bruising: Normal at injection sites. Usually resolves in 1–2 weeks
  • Brown staining (haemosiderin): A brownish discolouration along the treated vein occurs in up to 30% of patients. It fades over weeks to months. Sun avoidance and aspiration of trapped blood both reduce the risk.
  • Matting: A fine network of new tiny red vessels occasionally appears near treated areas. Usually resolves spontaneously or responds to additional treatment.
  • Vein still visible initially: Treated veins may appear unchanged or even more prominent for 2–4 weeks. They gradually fade over 4–12 weeks as the inflammatory reaction resolves and the vein fibroses.
  • Multiple sessions: For extensive thread veins, 2–4 sessions spaced 6–8 weeks apart give the best cosmetic outcome

✅ Your Follow-Up Appointment

Dr. Haggag schedules a follow-up assessment at 4–6 weeks after your session to evaluate the treated veins, identify any trapped blood that should be aspirated, and plan any additional sessions needed. Attending this appointment is important for achieving the best final cosmetic result.
